I would be interested in something like that. I started a clinical trial for migraines. They gave me a phone that had me fill out a daily log of when I had a migraine and the symptoms unfortunately the doctors office stopped the study and I have no way to get the information from the phone they gave me. It was very frustrating.

Right now I track and rate my migraines on my phone calendar. It's pretty easy and I give the number of migraines per month as well as any I rate 7 and over.
